Position
Title: Preferred Communities Intensive Case Manager
Reports to: Integrations Program Manager
Status: Full-Time/Exempt
Location: Silver Spring, MD
Job Summary: This position focuses on
providing intensive case management services to eligible refugees, asylees, Afghan SIV holders,
victims of trafficking, and secondary migrants. The Intensive Case Management Coordinator will facilitate the integration of newcomers into the host community, aiding them in accessing necessary services to achieve economic and personal self-sufficiency efficiently.
Key Responsibilities:
Holistic Case Management: Oversee and coordinate systematic, timely service delivery
tailored to address the diverse needs of community members, fostering cultural adjustment and self-sufficiency.
Needs Assessment and Service Planning:
Identify individual needs, develop comprehensive service plans, and track client progress, ensuring
effective support and program outcomes.
Referral Coordination: Facilitate appropriate referrals to social service agencies, community resources, and organizations, guiding clients in accessing vital services such as food stamps,
disability benefits, healthcare, housing, and language training programs.
Support Services: Provide practical assistance including transportation to medical appointments, mediation with landlords, assistance with medical paperwork, and crisis intervention as needed.
Healthcare Navigation: Educate and empower clients to navigate the healthcare system independently, offering guidance on appointment scheduling, communication with healthcare providers, and health-related paperwork management.
Collaborative Partnerships: Work collaboratively with external partners and organizations
to address clients’ intensive service needs, such as mental health and disability services, ensuring comprehensive support.
Cultural Orientation and Training:
Deliver one-on-one and group socio-cultural orientation sessions, covering essential topics for successful integration such as household management, health practices, budgeting, and community resources.
Language Support: Provide interpretation and translation services in refugee languages, ensuring effective communication and access to services for all clients.
Documentation and Reporting: Maintain accurate records of client interactions and service
delivery using agency forms and case notes, facilitating internal and external program evaluation and reporting.
Home Visits and Assessment: Conduct home visits to assess clients’ living situations, documenting findings in client case files to inform support strategies effectively.
Additional Duties: Undertake any other tasks as assigned by the supervisor to support the efficient functioning of the program.
Education, Experience, Knowledge, Skills and Abilities
• Bachelor’s degree in social work, public administration, or related field preferred.
• Previous experience in case management, record-keeping, or compliance-related roles preferred.
• Strong attention to detail and organizational skills.
• Excellent written and verbal communication skills.
• Ability to work independently and collaboratively in a fast-paced environment.
• Proficiency in Microsoft Office Suite and database management systems.
• Knowledge of refugee resettlement programs and grant funding regulations is a plus.
• Ability to maintain confidentiality and handle sensitive information with discretion.
• Ability to work independently and manage multiple tasks effectively in a fast-paced environment.
• A valid driver’s license and access to reliable transportation.
• Possess a valid driver’s license and access to own transportation
• Required computer skills: Word, Excel, Internet, Gmail, Outlook. Comfortable utilizing new programs and databases.
• Background check and clean driving record.
Licenses/Certifications: Must have a driver’s license and good driving record.
Physical Demands
• Moderate physical activity, which includes standing, sitting, lifting and/or walking.
• Ability to see within normal parameters.
• Ability to hear within normal parameters.
• Requires local travel to meetings.
• Ability to maintain emotional control under stress.
Environmental Factors: Work is primarily indoors in a climate-controlled building.
Contact with Others: Position involves regular contact with resettlement network affiliates; local,
state, and federal government agency employees, private sector organizations, ECDC/ACC staff and community members.
Other Information: All employees are expected to comply with ECDC’s employment
policies while carrying out their work.
