Role

The Heights and Hills Caregiver program provides social work support to informal caregivers, particularly caregivers of older adults, at no cost to our clients. Our team consists of four Caregiver Social Workers who each manage a caseload of ongoing clients, an Intake Social Worker, and a Community Outreach Social Worker.

The Caregiver Program Director is responsible for administration and supervision of the day-to-day operations of the Caregiver Program and provides administrative and supervision to staff and student interns. In addition, the Caregiver Program Director will represent the agency in community and interagency activities. The Caregiver Program Director works under the supervision of the Director of Programs. 

Responsibilities

  • Oversee screening and intake of all new potential caregiver clients.
  • Assign cases and related duties.
  • May provide social work services to a caseload of caregivers.
  • Provide supervision to caregiver staff and student interns.
  • Train new program staff and students.
  • Develop and cultivate community and corporate contacts.
  • Provide public presentations and training sessions on caregiving topics.
  • Oversee and monitor the provision of respite and supplemental services.
  • Perform monthly invoicing for the program, ensuring accuracy.
  • Review case records for clinical appropriateness as well as compliance to standards as established by NYC Aging.
  • Collaborate with Director of Programs to coordinate in-service training programs for staff.
  • Lead regular group supervision for caregiver staff.
  • Collaborate with the Executive Director and Executive Management team on outreach, program planning, and development.
  • Collaborate with the Director of Programs on special projects.
  • Compile statistics, prepare reports, and other required records.
  • Represent agency in community activities.
  • Perform other duties as required.

Required

  • MSW with a minimum of five years’ experience including two years’ supervisory experience.
  • Have an awareness of the biological, psychological, and social aspects of aging and the impact of disabilities and illnesses on aging and the family.
  • Excellent organizational skills and keen attention to detail.
  • Excellent clinical skills.
  • Ability to easily switch between diverse tasks in a fast-paced work environment.
  • Ability to arrange and negotiate service referrals.
  • Skills in establishing and sustaining interpersonal relationships.
  • Good presentation skills.
  • Proven ability to work both independently and as part of a team.
  • Must be able to work occasional evenings and weekend days.
  • SIFI-certified or eligible to obtain SIFI certification in first year.
  • Excellent computer literacy – Windows, Microsoft Office.
  • Must be able to travel on public transportation to do community presentations and home visits.
  • Must be able to climb steps to do home visits.

Preferred

  • Knowledge of NYC Aging services and programs
  • Knowledge of NYC Aging database (STARS)
  • Prefer bi-lingual and cultural competency in Spanish, Chinese or Haitian Creole
  • Knowledge of community resources and public benefits eligibility
  • Hospital or community experience working with older adults and/or their families.
